Thank you for your interest in the Jason Parker Pickleball Memorial Classic. The registration fee is $50 per person, and all participants must complete this form in full and agree to the Waiver & Release of Liability in order to secure their spot. The tournament will be held at the Long Beach Tennis Center in Long Beach, NY on Saturday, June 20th.
